Service Fees:
In-Office Individual, Couples, and Family Therapy Sessions (50-60 minutes) $ 100.00
Intensive In-Home Therapy Sessions per Visit (time varies) $ 125.00
Cash/Check payments upon each hourly visit or by other arrangement, as necessary.
Accepted.
Insurance Coverage:
PPO insurance invoices are available for reimbursement; however, all service fees must be paid at the time of each session, unless other arrangements have been made in advance.
Horizon EAP, VMC, and HealthCorp EAP accepted with pre-authorized reference number from customer care. No copay for EAP services.
I am willing to contract with county agencies, when necessary. Please have the patient representative contact me for more specific information. Be sure to call me first to insure I have space in my practice at the current time.
